The VNA of Hanover & Spring Grove is looking for experienced and hard-working Full-Time Registered Nurses to join our growing team in the Hanover, Spring Grove, and Adams County.

Oversee primary caseload of Home Health patients and provides skilled professional nursing care to them as prescribed by the physician and in compliance with the Home Health Medicare Conditions of Participation, PA licensure laws and agency policies and procedures. Provide the comprehensive nursing assessment of patient needs and collaborates to develop the patient plan of care. Implement the plan of care and reevaluates and revises the plan of care relative to the patient’s nursing needs to provide appropriate, comprehensive and responsive goals and interventions. Work closely with other members of the team to ensure all patient care needs are being met.

Primary Duties:

  1. Conduct comprehensive assessment of patient in a thorough, accurate, compassionate and timely fashion.
  2. Utilize case management techniques and the nursing body of knowledge to formulate and individualize the patient plan of care to include problem identification, interventions and goals.
  3. Contribute to identifying both active and potential needs of the patient which may affect the plan of care.
  4. Consistently collaborate with the physician to plan and implement the medical and nursing plan of care.
  5. Facilitate and participate in case conferences and contribute to patient care planning.
  6. The Registered Nurse is a mentor who is able to provide educational experiences and communicate knowledge of patient care to members of the team including, but not limited to: therapy staff, Home Health Aides, Medical Social Workers, patient and family/caregivers, health care professionals and the general community.
  7. The Registered Nurse is responsible for managing all patients assigned to his/her primary caseload by utilizing the following methods:

Qualifications:

  1. Graduate of an accredited registered nursing program.
  2. Bachelor’s degree in Nursing preferred.
  3. Has minimum of one year general nursing experience with current home health, medical, surgical or critical care experience preferred.
  4. Ability to work within a Home Health setting.
  5. Registered Nurse licensed in the Commonwealth of Pennsylvania.
  6. Valid driver’s license and reliable transportation that is insured in accordance with PA state requirements.
  7. Cardio Pulmonary Resuscitation (CPR) certified.
  8. Satisfactory references from employers and/or professional peers.
  9. Satisfactory criminal background check.

Works closely with other members of the interdisciplinary team to ensure all patient and family care needs are being met.

Qualifications:

  • Has minimum of one year general nursing preferred with current hospice, home health, medical, surgical or critical care experience.
  • Experience with pain and symptom management interventions preferred.
  • Ability to work within an interdisciplinary setting.
  • Understanding of Hospice philosophy, principles of death/dying.

Education, Licensure, Registration, and/or Certification:

  • Registered Nurse licensed in the Commonwealth of Pennsylvania.

Home Health LPN - $5,000 Sign-On Bonus (one year commitment)

Sign-on Bonus $5,000 - one year commitment

The VNA of Hanover & Spring Grove is seeking a full-time, day shift Licensed Practical Nurse(LPN). 

Under direction of the RN Case Manager the Home Health LPN provides skilled nursing care to Home Health patients in their home. Must have the ability to work in a home health setting and participate in a weekend rotation.

Qualifications:

  • A minimum of one year experience is required.
  • Administers nursing care in a home care environment under the supervision of a registered nurse.
  • Participates in the implementation and evaluation of patient care
  • Ensures the health, comfort and safety of patients

Education, Licensure, Registration, and/or Certification:

  • Active PA LPN License
  • Valid driver’s license and reliable transportation that is insured in accordance with PA state requirements.
  • Graduate of an accredited practical nursing program

Insurance Verification Specialist

The VNA of Hanover & Spring Grove is looking for experienced and hard-working Full-Time Insurance Verification Specialist to join our growing team!

This position would perform insurance benefit verifications and completes authorizations for patients referred to Home Health and Hospice services.  Cross-trains with the Billing/Accounts Receivable Specialist and other positions and/or duties in the Finance department.

Primary Duties:

  • Perform benefit verifications for Home Health and Hospice admissions.
  • Obtain authorizations for Home Health and Hospice admissions.
  • Maintain the authorization tracking form to track visit counts and beginning and ending dates of authorizations by discipline and perform follow-up to obtain additional authorizations as needed.
  • Monitor visit data and Physician Orders in the back office to assure proper authorizations are obtained.
  • Review patient demographic data for all Home Health and Hospice admissions to verify data is accurate and up-to-date.
  • Prepare and send Home Health benefit letters for new admissions.
  • Perform backup of essential duties for Billing/Accounts Receivable Specialist.
  • Perform backup to front desk receptionist as required.

Secondary Duties:

  • Perform any other duties as may be assigned by Director of Finance.
  • Support the essential process of sustaining and improving performance.
  • Mandatory participation in staff meetings, departmental meetings, and in-services.
  • Participate in committees and other related activities as required.

Qualifications:

  • Associate Degree in accounting related field desired.  High school diploma or equivalent required.
  • A minimum two years of experience in insurance authorization and benefit verification highly preferred.
  • Experience in medical billing and accounts receivables highly preferred.
  • Ability to work with a high attention to detail and multitasking.
  • Ability to understand policies and programs of the Agency.
  • Ability to understand and practice principles of confidentiality.
  • Experience with computer and Microsoft Office Suite in a Windows environment.  Strong Excel skills desired.

PRN Speech Therapist

The VNA of Hanover and Spring Grove is searching for a Speech Therapist to provide professional services to patients in the comfort of their home.  Part-time as needed (PRN) position available, flexible hours needed.

Qualifications:

  • Bachelor's or Master’s degree in speech therapy approved by an accredited program, current PA license, and minimum two years experience.
    Ability to work in a home health setting
  • A valid driver’s license with reliable transportation
  • CPR certification required.
