Director of Clinical Services, Home Health and Home Care Plus
As a member of the Senior Leadership Team, the Director of Clinical Services is responsible for the oversight of all personnel and agency day-to-day operations and provision of services that are provided by the home health and home care agency, whether directly or under arrangement. The Director of Clinical Services ensures quality and safe delivery of home health and home care services; coordinates patient related services that reflect the agency’s philosophy and high standards of care; and plans, develops, implements and evaluates agency quality services, programs and activities.
This professional demonstrates flexibility in meeting the agency’s needs, assumes delegated responsibilities from the President and CEO for operational activities that relate to quality, cost, and outcomes of patient care and is clinically in charge of the home health and home care plus departments. Education and Experience: Current license as a PA Registered Nurse, who holds a minimum of an undergraduate degree and has experience in health service administration, with strong supervisory or administrative experience in home health care or a related health care program.
Minimum of five years clinical nursing experience. Qualified to operate a home health agency in the day to day operations. Has the ability to communicate effectively with agency administration, executive team, governing board, interagency departments and external resources.
Hospice Clinical Manager - Full time position
As a member of the Management Team, the Hospice Clinical Manager is responsible for coordinating and providing quality patient care and responsible for management of nursing staff. This position requires extensive knowledge relative to Medicare, Medical Assistance, and third-party reimbursement requirements, accrediting and regulatory body regulations relative to the provision of Hospice services. This professional demonstrates flexibility in meeting the agency’s needs, assumes delegated responsibilities from the Hospice Clinical Officer for operational activities that relate to quality and outcomes of patient care. Education and Experience: Bachelor of Science in Nursing, equivalency, or candidacy preferred. Minimum of three (3) years clinical nursing experience in hospice preferred with a sound knowledge base. Supervisory experience highly preferred. Current license as a Registered Nurse in the Commonwealth of Pennsylvania. (CPR) certified. Must demonstrate strong clinical competency. Hospice Certification within two (2) years is recommended.
HCP Aide - Part-time/PRN
The VNA of Hanover and Spring Grove is searching for home care aides to provide personal care services to clients in their home. The home care aides assist clients with personal care, meal preparation, light housekeeping, companionship, running errands, etc. Qualified candidates must hold a CNA certification or be able to provide proof of nurse aide training. A minimum of one-year work experience is required. The ability to work in a home setting and reliable transportation is necessary. Mileage reimbursement offered, and paid travel time offered.
PRN Spiritual Counselor
Provides spiritual counseling services to Hospice patients, families and caregivers. Must be ordained, consecrated, commissioned and/or licensed and in current good standing with appropriate religious body. A degree in divinity, theology, spiritual care, ministerial studies, psychology, sociology or social work; a minimum of 1 to 2 C.P.E.’s from an accredited program; and experience in grief or loss counseling required. A minimum of two years’ experience in home health care preferred.
