Registered Nurses for Home Health and Hospice Agency. VNA of Hanover & Spring Grove is looking for experienced and hard-working Full-Time and Part-Time Registered Nurses to join our growing team in the Hanover, Spring Grove, and Adams County.

In this role some of your key responsibilities would include but are not limited to:• Serve as the leader of the patient care team. • You will assess your patients and establishes a plan of care engaging the patient and/or family in the process. • As a part of your duties, you will also implement the plan of care, including patient and family education, in collaboration with other members of the team. • Coordinate the patient's care with other disciplines and within the nursing care team. • Clearly communicates expectations to other members of the nursing care team, delegates tasks appropriately, and follows up to ensure that tasks are completed. • Assesses the patient's' response to the plan of care, informs MD of changes in the patient's condition, and alters the plan of care as needed. • Responsible for skilled nursing care to patients at home under the direction of and in accordance with physician's plan.
Qualified applicants must possess an active PA RN license, experience in nursing, and CPR certification required. Good computer skills, reliable transportation with active driver's license, and the ability to work in a home setting are necessary. Strong organizational, time management, customer service, and teamwork skills a must!
Hospice RN - $5000 Sign-On Bonus (one year commitment)
The VNA of Hanover and Spring Grove is searching for an experienced Registered Nurses for our Hospice program! Oversees primary caseload of hospice patients and provides skilled professional nursing care to them, their families and caregivers as prescribed by the physician and in compliance with the Hospice Medicare Conditions of Participation, PA hospice licensure laws and agency policies and procedures.
Provides the initial assessment and comprehensive nursing assessment of patient needs and collaborates to develop the Interdisciplinary Group plan of care. Implements the plan of care and reevaluates and revises the Plan of Care relative to the patient’s nursing needs to provide appropriate, comprehensive and responsive goals and interventions. Works closely with other members of the interdisciplinary team to ensure all patient and family care needs are being met.
Current RN licensure in PA and general nursing experience in medical, surgical, critical care, home health, hospice or oncology required. Qualified candidates must possess compassion and excellent organizational, time management, teamwork, and customer service skills! Ability to work in a home setting necessary! CPR, valid driver's license, and reliable transportation also required.
Occupational Therapist - $5000 Sign-On Bonus (one year commitment)
The Occupational Therapist (OT) provides occupational therapy for home health patients in accordance with the individual patient’s needs and goals as addressed in the plan of care. The OT must have knowledge and understanding of a patient’s illness or injury and the ability to assess these needs and plan appropriate interventions. The role of the occupational therapist is to work as part of the home care team including RN, PT, SLP, MSW and Home Health/Hospice Aides to assist the patient in achieving their optimum goals. The Occupational Therapist also provides palliative occupational therapy services to VNA Hospice patients on an as-needed basis, in accordance with the hospice plan of care.
- Current Occupational Therapy license in State of Pennsylvania.
- Basic Life Support certification.
- Pennsylvania driver's license.
- Proof of current auto liability insurance.
Preferred Position Qualifications:
- Home Health experience.
- 1 year experience as an Occupational Therapist.
Provides spiritual counseling services to Hospice patients, families and caregivers. Must be ordained, consecrated, commissioned and/or licensed and in current good standing with appropriate religious body. A degree in divinity, theology, spiritual care, ministerial studies, psychology, sociology or social work; a minimum of 1 to 2 C.P.E.’s from an accredited program; and experience in grief or loss counseling required. A minimum of two years’ experience in home health care preferred.
PRN Speech Therapist
The VNA of Hanover and Spring Grove is searching for a Speech Therapist to provide professional services to patients in the comfort of their home. Part-time as needed (PRN) position available, flexible hours needed. Qualified candidates must have a Bachelor's or Master’s degree in speech therapy approved by an accredited program, current PA license, and minimum two years experience.
Ability to work in a home health setting, a valid driver’s license with reliable transportation, and CPR certification required.
